Membership Applications                 

Firms interested in NAPSLO membership must submit a completed application and related materials (See application for list of required materials). Please select the appropriate application from the following section to download. Adobe Acrobat Reader, available free from Adobe, is required to view the application. 

The NAPSLO Board approved several changes in June 2006 regarding membership criteria. Please review the criteria pages of the website or the criteria section of the application packet.

NOTE: Please complete the application that appropriately reflects the structure and focus of your organization. Application files  below are large PDF files and may take a minute to download. 

Reference forms are included in the application packets below, or may be downloaded separately. Reference forms must be signed by the individual submitting the form.

Wholesale Brokers are sole proprietorships, partnerships or corporations licensed in at least one jurisdiction within the United States or Canada as a broker, agent or managing general agent engaged in an insurance business principally as a wholesaler.

Company members are risk-bearing insurer incorporated or organized in any state in the United States or in Canada and engaged in the writing of insurance principally through wholesale brokers/agents.

Underwriting Managers have specific underwriting authority for one or more companies in specified classes of business and in designated geographical areas.

Associates are individuals, firms or organizations that do not qualify for voting membership, but supply services to voting members and supports the wholesale excess/surplus lines system, or a company or wholesale broker/agent who is not incorporated or organized in any state in the United States or in Canada. 

Please review the entire application and note what additional items are required to be submitted to complete the application file. Incomplete files will not be reviewed.           

Deadlines  to submit completed application files are:    

January 22, 2010 for review in March 2010

April 30, 2010 for review in June 2010

August 27, 2010 for review in October 2010
